TSG worked with the Arkansas Department of Health, Crittenden County and FindHelp to develop a grant proposal to the Centers for Disease Control for their 2021 national initiative called the Social Determinants of Health Accelerator Plans. The purpose of the grant was to develop a multisector implementation-ready action plan that address the social determinants of health by accelerating action in Crittenden County that will lead to improved chronic disease outcomes among persons experiencing health disparities and inequities. Crittenden County was selected as one of ten States or Local Governments across the country that received the grant and the TSG team has worked with the multisector leadership team to implement the Plan that will used by CDC to develop future strategies to reduce health disparity and promote health equity.
